Now hiring for a Full-Time RN Clinical Admissions Coordinator!
Essential Job Functions Include:
- Complete a pre-admission screening to provide a complete medical assessment.
- Collect medical history of the patient.
- Respond to inquiries from families, Hospital Social Workers, Discharge Planners, and other referral sources.
- Communicate with Director of Nursing, Nursing, Administrator, Social Work, and Admissions Coordinator regarding potential SNF applicants and any additional duties for admission to facility.
- Interview and educate Residents and families on treatment and care prior to admission to the facility.
- Coordinate with nursing staff (clinical/administrative) to ensure comprehensive Resident plan of care is being created.
- Assist in a periodic review of materials utilized to help applicants and families prepare for admission.
- Coordinate arrival of new resident.
- Assist with discharge planning.
- Contribute ideas for efficacy and improved systems.
- Develop and maintain a good working rapport with inter-department personnel to ensure that the needs of residents are met.
- Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices
- Additional duties as assigned by the Administrator and Director of Nursing.
Requirements:
- Must possess a current NYS RN license.
- CPR and BLS certifications required.
- Minimum, associate’s degree from an accredited program.
- Minimum, two year experience in health care management.
- Experience in a geriatric setting preferred.
Qualifications:
- Must possess critical thinking and organization skills.
- Must be compassionate and emotionally stable to handle difficult situations at times with Residents.
- Must be able to advocate for seniors and families, as well as effectively communicate with them.
- Must possess basic to intermediate Computer skills.
- Must be able to read, write, speak and understand the English language in an understandable manner.
- Must be able to perform essential job functions (reasonable accommodation may apply.)
- Must be able to function independently, have flexibility, personal integrity and the ability to work effectively with Residents, staff, visitors, volunteers, government agencies/personnel, etc. under all conditions and circumstances.
Must be vaccinated against COVID-19 per NYS regulations. Masks required for all staff members.
