RN Case Manager Care at Home
Monday-Friday 8a-5p
1.0 FTE Full Time Benefits, 40 hours/week
The position is providing direct patient care in the home as an extension of the clinic. This position is more aligned to a clinic visit but will go to the home. The position encompasses a variety of population health services-clinic based services-that we provide in the home. Hospital to home, post-discharge home visit, annual wellness visits at Home. Advanced care planning at Home, etc. Full RN scope of work delivered in the home as an extension of our clinic. These are population health programs. As a service line we get the patients primarily through our collaborative work with the inteli-centers-varied referral sources- a lot from EDs, Clinics, and maybe some from the hospital as well but that's not our target referral. Our goal is truly for the patients to not be in the hospitals, more primary care and urgent care clinics. This is very innovative and transformational care delivery, so this would be at forefront of that work. Document primarily in Epic ambulatory (Clinic EHR) along with operational entry into McKesson. We are not managing the care, but instead engaging with the patient to coach the patient in self engagement and self care-not to go in and deliver care. Hospital to home we are delivering high acute care. It will be a mixture of both. We are working with very varied populations from very low risk to very high acuity to very stable.
Education: Graduate of State Board approved program for Registered Nurses. Complete and maintain UPH Person-Centered Care: Three opportunities completed per year required (Oct – Sept scheduled year). Attend all mandatory education programs.
Experience: Strong interpersonal skills. Ability to work as a collaborative team member. Ability to understand and apply guidelines, policies and procedures. Strong computer skills.
License(s)/Certification(s): Valid / maintained license as a Registered Nurse for all states applicable to practice. Valid / maintained Basic Life Support (BLS) Healthcare Provider Card with Re-certification. Valid / maintained Mandatory Reporter course by state(s) requirement. Valid / maintained driver’s license and automobile insurance in accordance with state and/or organizational requirements.
Knowledge/Skills/Abilities: Writes, reads, comprehends and speaks fluent English. Multicultural sensitivity. Microsoft Office – basic computer skills. Customer/patient focused. Critical thinking skills using independent judgment in making decisions

Pay: $63,336.00 - $94,993.60 per year
