Overview:
The Iowa Department of Public Health (IDPH) and Department of Human Services (DHS) merged on July 1, 2022, to form one agency, the Iowa Department of Health and Human Services (HHS). HHS Targeted Case Management (TCM) is seeking a Disability Services Case Manager (Social Worker 2). This position will be based in Des Moines and will serve a multi county area, helping individuals with disabilities access resources they need to thrive.
Hours and Travel:
Hours for this position vary based on the needs of the individuals we support, Monday - Friday. Travel is based on the needs of the individuals supported and in-home visit are required. Mileage is paid at $0.50/mile travelled according to the Department of Administrative Services (DAS) State Accounting Enterprise Procedures. There is very limited overnight travel.
Special Requirements:
A valid driver's license and a reliable vehicle is required for this position.
Core Job Duties:
The Disability Services Case Manager works with individuals with Intellectual Disabilities (ID), Developmental Disabilities (DD), and individuals utilizing the Brain Injury (BI), Health and Disability (HD), Physical Disability (PD), Elderly and AIDS/HIV waivers. They help identify service needs, obtaining them, and evaluate progress in the provision of those services.
In addition, the Disability Services Case Manager will:
- develop individual care plans and service coordination across agencies.
- assist individuals and/or guardians navigate the service system, secure crisis services, find new services and track outcomes.
- ensure the individuals needs are met in a way that allows them to remain independent in their community and participate in decisions regarding their service plan.
- previous experience as a Case Manager and a strong understanding of the disabilities listed above.
- sound judgment and the ability to evaluate situations with accuracy and compassion.
- excellent communications skills and the ability to modify communication methods to suit the individuals supported.
- ability to correctly interpret laws, rules, regulations, policies, and procedures.
- are experienced in using Microsoft Office programs and/or electronic medical records.
- have interviewing and team building experience.

920 Case Management
Bachelor's Degree with at least 30 semester hours including:
Behavioral Science, Education, Health Care, Human Services Administration, Social Sciences
Applicant must also possess 1 year experience in delivery in one or in a combination of the following areas of human services: Chronic mental illness, Developmental disabilities, Intellectual disabilities
OR Applicant must have Iowa license to practice as a registered nurse AND the equivalent of 3 years nursing or human services experience.Graduation from an accredited four-year college or university;
