As a Care Management Nurse, you will work with Regional Medical Directors, Health Care Programs, and outside healthcare providers to ensure that patients receive necessary care in a timely and appropriate manner. You will use our EMR system, health records, and your clinical expertise to facilitate care coordination, and communicate outcomes as needed. Your primary goal will be to improve patient outcomes by ensuring that they receive high-quality, patient-centered care.
- Collaborate with the Director of Care Management (DCM) to identify and resolve care issues, improve the Care Management process, and ensure the delivery of medically necessary services.
- Work with assigned regions to manage inpatient stays, identify cost alerts, perform reinsurance notifications, and facilitate discharge planning for patients.
- Communicate regularly with hospital utilization review departments and consult with Regional Medical Directors (RMD) and DCM as needed to address any concerns.
- Identify areas of concern and assist in the review and improvement of the Care Management program and process.
- Ensure appropriate documentation of all Care Management activities in the EMR system and adhere to relevant post orders and site contracts.
Education
- Graduation from an accredited School of Nursing
Experience
- Minimum 1 year of experience of care delivery in a managed care environment.
- Computer data entry experience in MS Word, Excel and PowerPoint.
- Previous experience working with Care Management (CM).
- Chronic care experience highly preferred.
- Understanding of care delivery in a correctional environment preferred.
Licenses/Certifications
- A current nursing license in the state of employment required.
- Must be able to attain and maintain security clearance to the facility.
