Responsible for the Home Health program at their location. This includes planning, organizing, coordinating, and evaluating all clinical and administrative operations.
ESSENTIAL DUTIES AND RESPONSIBILITES
- Directs the daily clinical and administrative activities of the Home Health program.
- Provides leadership, direction, and supervision for the Home Health staff.
- Reviews, revises, and develops administrative and clinical policies and procedures to be approved by the organization.
- Ensures compliance with Medicare Conditions of Participation and Georgia State Regulations.
- Develops Home Health goals and objectives under the direction of the Director in accordance with the mission of Home Health.
- Acts as a liaison between the Governing Body, Medical Staff, and Home Health staff.
- Oversees Home Health quality assessment/performance improvement activities in conjunction with the Quality Assessment/Performance Improvement Team.
- Conducts ongoing community Home Health needs assessment, and participates in the development of new Home Health-related programs and services.
- Assists in the negotiation and coordination of contractual agreements with pharmacies, medical equipment companies, and supplemental services.
- Establishes and maintains physician relationships, ensuring continuity of care across the Home Health referral network.
- Provides information to and serves as liaison between physicians, community agencies, social service departments, nursing homes, acute care facilities, and other agencies.
- Supports the preparation of the yearly Home Health budget for approval by the Governing Body. Assures financial success and sustainability by closely monitoring expenses and revenues.
- Oversees the recruitment, selection, promotion, and termination of Home Health personnel, ensuring that qualified staff is hired and that staffing needs are met. Ensures that complete and accurate personnel records are maintained.
- Conducts annual performance appraisals on all Home Health administrative staff and reviews appraisals of all clinical staff.
- Assists staff in understanding corporate mission and department's goals and objectives, to include supporting the development, conduct and scheduling of in-services.
- Oversees the associate orientation program to ensure that associates are oriented trained and supervised; and ensures the completion of annual training and supervision programs.
- Maintains and promotes an open and supportive environment.
- Ensures/implements/promotes/supports quality assessment/performance improvement activities, evaluations, and improved outcomes.
- Continually looks for ways to improve workflows and increase efficiencies with processes.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
- Performs other duties as necessary to ensure the success of the System.
SKILLS AND ABILITIES
- This position requires extensive knowledge of Home Health. Strong leadership qualities and experience in Home Health program development and fiscal management required.
- Extensive knowledge of healthcare and Home Health care organizations and their structure.
- Considerable ability to plan and direct subordinate staff in the execution of their activities and prioritize workload.
- Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
- Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization presidents and their leadership teams, and the general public.
MINIMUM QUALIFICATIONS
- Is a licensed physician; or a registered nurse; or has training and experience in health service administration and at least one year of supervisory or administrative experience in Home Health or related health programs.
- Ability to meet the organization's conditions of employment regarding health clearance, and provisions of professional references.
