Southside United HDFC, better known as Los Sures, is a multi-service, community based non-profit organization based in Williamsburg, Brooklyn. Our company provides a wide range of services to our tenants and community members ranging from affordable housing and tenant organizing, to Social Services and activities for older adults.
Position: Senior Tenant Service Coordinator
The Senior Tenant Service Coordinator will report to the Social Services Director. The Senior Tenant Service Coordinator will be responsible for providing service coordination to formerly homeless older adults as well as other tenants in the senior building. His/her responsibilities will primarily include:
· Actively coordinate day-to-day on-site services for tenants
· Supervise on-site p/t Tenant Coordinator
· Conduct needs assessments of tenants to determine their wellness and social needs.
· Provide ongoing case management, assistance, and advocacy, providing information and referrals to community services as needed, as well as supportive counseling and crisis intervention services.
· Ensure tenants maintain the housing subsidy and the benefits to remain in permanent housing.
· Help tenants identify additional benefits for which they may qualify and facilitate application to said benefits, conducting advocacy as needed.
· Document contact with tenants, their families, and providers.
· Conduct monitoring services through home visits and telephone calls
· Educate tenants on the available on-site services and other community resources.
· Assist tenants in developing informal support networks with other tenants by facilitating and organizing opportunities for tenant engagement through educational workshops and social activities including holiday celebrations.
· Organize and conduct outreach for educational workshops based on residents’ interests and needs; provide translation as needed.
· Communicate with property management and maintenance staff on tenant issues.
· Work collaboratively with community partners to ensure residents can age in place, including coordinating homecare services and proper discharge planning from inpatient facilities.
· Work as part of the Social Services team & Other related duties as needed.
Qualifications:
- Bachelor's degree in human services or related field is required
- Proficiency in Spanish is a must
- Experience working with low-income population and seniors
- Knowledge of public benefits system and supportive services for seniors and others
- Strong organizational, time management, and communication skills
- Ability to problem solve and work with a high level of autonomy required
- Ability to be a team player and willingness to work with a diverse population
