Job Description
We're seeking an RN to serve as Field Supervisor. The Field Supervisor is responsible for assisting with staff education and training/development, including: continuing education, in-services, and training (in house or in the field) to further develop and maintain competence.
Job Functions/Responsibilities:
Knowledge of OASIS guidance, interpretation, and time points
Knowledge of CHHA regulatory guidelines per center for Medicare and Medicaid, New York State Department of Health (DOH), and Joint Commission (JC)
Assist with orientation of the new staff on policies and procedures
Accompany the new staff into the field post orientation, conduct random field visits, complete annual performance evaluations, and accompany the staff during survey visits
Knowledge of Strategic Healthcare Programs (SHP) reports; utilize and conduct for staff performance
Assist in the screening and interviewing process of new organization personnel and make recommendations for employment of individuals; assist in the orientation of new organization personnel
Assist the educator in the planning, implementation, and evaluation of in-service and continuing education programs
Review patient’s clinical diagnosis, medications, procedures, and clinical course
Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care
Participate in EMR software upgrade, testing, and staff education
Participate in Quality Assurance Performance Improvement (QAPI) activities
Participate in the agency’s emergency preparedness plan
Comply with acceptable professional standards and practice
Requirements:
- Must be a Registered Nurse in the state of New York
- Must have two (2) years of supervisory experience and home care experience with a CHAA
- Computer experience required
- Bachelor's degree preferred
