Summary/Objective: The Registered Nurse plans, coordinates, implements, evaluates home health services, and is experienced in nursing, with an emphasis on community health education. The professional nurse builds from the resources of the Organization & community to provide services to meet the needs of individuals and families within their homes. The Registered Nurse is responsible for cost efficient and effective patient care with quality outcomes Essential Duties and Responsibilities: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Completes an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs. Provides a complete physical assessment and history of previous illness(s) and uses the health assessment data to determine appropriate diagnosis, and need for other disciplines and resources.Develops an individualized care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Makes necessary revisions to the care plan as patient status and needs change. Completes and updates the ongoing assessments, and care plan as patient’s status changes.Initiates appropriate preventive and rehabilitative nursing procedures and treatments. Administers medications and treatments as prescribed by the physician.Ensures that arrangements for equipment and other necessary items and services are available.Provides health care instructions and education to the patient and or family as appropriate per assessment plan and patients’ caregivers’ ability to learn. Adapts teaching styles to meet patients and family’s needs.Identifies discharge-planning needs as part of the initial care plan development and implements prior to discharge of the patient.Develops, monitors, and updates the HCA plan of care as necessary including completion of HCA supervisory visits no later than every 14 days. The HCA plan of care is complete, accurate, and updated every certification or as patient needs change.Updates the primary physician when necessary and at least every sixty days.Communicates with the physician regarding the patient’s needs, reports, and changes in the patient’s condition; obtains/receives physician’s orders as required.Communicates with all members of the healthcare team to promote coordinated, efficient care and documents coordination of care with all involved with patient care.Initial assessments are complete, accurate, and submitted within 48-72 hours of admit. Additionally, ongoing clinical documentation is complete, accurate, documented, and submitted within 24 hours of patient visitParticipates in weeknight and weekend on-call duties as defined by the on-call policy. Position Qualifications: Education: Graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon. Experience: One to two years of recent medical/surgical care experience in an institutional setting. Ideal candidates will possess 1-2 years of home health experience. Experience/ability to work as part of an Interdisciplinary Team. Licensure / Certification:Valid State of Oregon School of Nursing LicenseCurrent automobile insurance required. Valid Oregon driver’s license and/or reliable transportation required.Current CPR required Knowledge/Skills/Abilities:Knowledge of Federal, State and Local regulations and Medicare Conditions of ParticipationKnowledge of HIPAA privacy rules and regulationsKnowledge of electronic medical records systemsExpertise in discharge planningKnowledge of Home Health ServicesDemonstrate basic data entry skillsDemonstrate excellent organization skillsDemonstrate excellent interpersonal skillsDemonstrate excellent written and oral communication skillsAbility to work under pressure to meet deadlinesAbility to take initiative and utilize innovative techniques and ingenuityAbility to interact with various departments throughout the organizationAbility to work flexible hours, if neededAbility to participate as a team player Ability to plan, prioritize and coordinate multiple projects Environmental / Working Conditions: May be exposed to extremes of heat and cold in all weather conditions. May drive in various weather conditions on roads in varying degrees of repair.May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required throughout Madras and surrounding areas.OSHA exposure Category I: Position includes tasks that involve exposure to blood, body fluids, tissues. Required Personal Protective Equipment as conditions warrant including: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap. THE ORGANIZATION MAINTAINS ITS STATUS AS AN AT-WILL EMPLOYEE Schedule: Full time position (32-40 hours week) - normal shifts are Monday through Friday 8am – 5pm. You may be called upon to flex your hours (up or down) to meet the needs of the business. Additionally, you may be required to assist in providing on-call coverage when necessary, and as assigned by the Supervisor. Compensation: $32.00 - $48.50 Hour DOE Benefits: Full time employees who successfully complete their first 60 days of employment are eligible for benefits which include Health, Dental, Vision, Paid-time off (vacation, holiday, and sick pay) 403(b) retirement plan (employee contribution) and Life Insurance.
