Position Summary
The Nurse Case Manager will administer medical case management services to clients enrolled in the Supportive Housing Program in Jackson County. If the full-time option for this position is desired by candidate, the position would additionally travel to serve clients in Douglas and Josephine Counties. The Supportive Housing program provides intensive wrap-around supports to individuals facing significant barriers to obtaining and maintaining stable housing, often with a history of behavioral health and/or substance use barriers. The ideal candidate will be comfortable working with these populations. The Nurse Case Manager will often travel to deliver medical case management in the home in order to further reduce clients’ barriers to accessing care. The Nurse Case Manager will promote viral suppression and increased overall wellness and stability for Supportive Housing clients.
The Nurse Case Manager will work in close partnership with HIV medical providers in the community. This position will communicate with providers regarding the client’s case management care plan and the medical treatment plan. In addition, HIV medical Case Managers administer services that many HIV doctors have limited time to provide: treatment management and medication adherence counseling, HIV disease and side effects management, nutritional therapy, oral health interventions and referral, liver health and hepatitis disease management (acute and chronic), HIV risk reduction, STI screening and sexual health education.

Health Resources Services Administration (HRSA) defines medical case management as:
“Medical case management services (including treatment adherence) are a range of client-centered services that link clients with health care, psychosocial, and other services. The coordination and follow-up of medical treatments is a component of medical case management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family members’ needs and personal support systems. Medical case management includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
Key activities include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) provision of services required to implement the plan; (4) monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client. It includes client-specific advocacy and/or review of utilization of services. This includes all types of case management including face-to-face, phone contact, and any other form of communication.”

Essential Functions & Responsibilities

Provide frequent in-home delivery of services and/or medication, as determined by client need
Travel to conduct home visits and appointments with clients in Lane, Douglas, and Josephine Counties
Complete regular health assessment on eligible clients
Develop and lead RN care plan for eligible clients
Conduct case conferences with Ryan White Care coordinators
Conduct case conference with medical providers, including HIV specialty care
Build and maintain relationships with medical providers
Complete annual HIV RN training
Advocate for clients with medical service providers
Provide direct one-on-one medical case management services to eligible clients
Maintain up-to-date knowledge of disease progression and relevant therapies
Provide client education pertaining to disease progression and relevant therapies
Maintain up-to-date and accurate client records
Advocate with other Case Managers and other service providers on behalf of our clients
Provide intensive, short-term intervention to newly diagnosed clients focusing on HIV disease management, prevention/risk reduction education
Participate in necessary meetings
Assist Director/Manager with all necessary reports
Participate in agency teams
STI testing including specimen collection, STI diagnosis and results delivery
Clinic planning and oversight
Assisting with training and supervising RN volunteers
Other duties as defined by Director and Ryan White Principles and Standards

Required Qualifications

Valid Oregon Registered Nursing license
Valid Oregon driver’s license, driving record sufficient to be covered by agency auto insurance policy, ability to frequently transport self to job-related trainings and client homes
Ability to pass agency criminal background check
Ability to work effectively with challenging patients
Ability to pass AIDS RN Certification within 6 months
TB test provided by agency

Preferred Qualifications

Bilingual in English and Spanish Preferred

Physical Demands
While performing the duties of this job, the employee is required

  • Repetitive use of hands such as grasping, twisting/turning of wrists, and finger dexterity necessary to perform various clerical duties such as typing, writing and maintaining files.
  • Frequent use of arms such as pushing/pulling to open and close drawers and cabinets.
  • Ability to sit for extended periods of time at assigned station.
  • Ability to bend and/or reach, and to occasionally lift and carry up to 25 pounds (boxes of copy paper and/or files for storage).
  • Continuous clarity of vision at 20 inches or less for normal processing of work

Working Conditions, Travel and Environment
The duties of this job require regular attendance

  • Ability to work flexible hours, including early mornings, evenings, and weekends
  • Ability to work 5 days per week for a minimum of 20 - 40 hours per week

Travel to other locations as necessary to complete essential job functions

Ability to transport self to job-related events and locations
