To provide nutrition education, counseling and interventions as appropriate, in the management of nutritional status and disease management for residents to their disease states and therapies.
This position is responsible for supporting, understanding, interpreting, and implementing the philosophy and policies of the Facility.
Assists the facility by directing staff towards obtaining results in the following area(s): quality, patient satisfaction, business management, teamwork, culture and employee satisfaction.
DUTIES:
Coordinates with the Dietary Manager and Diet Tech in completing Nutritional Assessments - initial, quarterly, annually and any significant changes
Collaborates with physician and/or NP designee for coordination of care to meet nutritional needs and diet order for residents
Monitors progress of all residents, including but not limited to: monthly weights, lab values, intake, tube feeds, pressure ulcers, dialysis, and other high risk factors
Provide recommendations interventions as needed and indicated
Conducts education, training and in-servicing with staff, residents and family on meal planning, nutrition, & diet education
MINIMUM QUALIFICATIONS:
1. Master’s degree required.
2. Registered Dietitian as per Commission on Dietetic Registration
3. Dietitian State licensure (RDN) in the State of Illinois
3. One year experience in clinical nutrition as a Registered Dietitian (preferred)
4. Computer experience preferred (experience with google drive)
5. Must be able to travel to buildings that are designated to the employee to provide on-site services per company policy
