Job Description
Responsibilities:
- Perform comprehensive screening of client documents and referral forms to determine eligibility into Links2Health program
- Referral Submissions and Tracking
- Maintain accurate and up-to-date records of all incoming referrals, ensuring that each referral is properly documented
- Complete and submit referrals for authorization
- Track referral metrics according to program designation
- Follow up on denied or voided authorizations
- Reconnection to local Medi-Cal benefits
- Connect to services via closed loop referral portal
- Monthly caseload organization and verification
- Verifies member eligibility via Alliance Provider Portal on a monthly basis for all caseload members
- Close collaboration with Program Manager and Program Coordinator
Qualifications:
- 2+ years of experience working with disenfranchised populations, SUD, Unhoused, and those with serious mental illnesses.
- HS Diploma or GED or equivalent required
- Strong computer skills, including able to easily navigate around health care systems
- Preferred: Bilingual/bicultural
Wages & Benefits:
Our total compensation package includes a comprehensive medical and dental insurance plan for full time employees, 403(b) retirement plan, paid holiday, sick time, and 17 days of paid vacation, pro-rated for positions less than 40 hours a week.
