Job Summary:
The Quality Specialist (SNF) will be responsible for managing the collection, organization, input and presentation of the Quality Incentive Payment Program (QIPP) with proven quality outcomes for the company. This position will identify areas of focus to effect improvement and change to meet QIPP metrics.
Essential Functions:
- Manage the QIPP program for 37 SNFs in Texas
- Monitor the Quality Measure ratings for all SNFs every month and develop a specific plan for improvement on any QM above the National percentage or over the 75th percentile with emphasis on the 4 Quality Measures that impact the QIPP program.
- The QM reports including the facility characteristics and the resident roster out of Simple LTC will be analyzed to determine the appropriate interventions for each facility
- Education focused on QM to include managing the MDS process to enhance the QM's and reimbursement related to the MDS
- Review the facility transmission reports through Simple LTC to:
a. Evaluate the errors and assist the facility with correction, if appropriate
b. Educate the MDS Coordinator/DON related to the errors/issues
c. Assist with the corrections to the MDS based on the resident assessment to increase accuracy of the MDS, decrease triggers, and improve reimbursement - Provide focused training with the facilities who have difficulty managing the QM process, have a new MDS Coordinator or DON, or at the request of the administrative staff or corporate staff.
- Provide QM analysis reports to the corporate team every month with an action plan for any facility above the national percentage or above the 75th percentile on any QM
- Manage all aspects of the QIPP initiative ensuring metrics are meeting expectations and the reporting requirements are met
- Responsible to drive the Quality Measures metric offering strong outcomes that will influence the 5 Star rating
Continuing Education:
- Attends in-service and continuing education programs required for maintenance of professional certification or licensure
- Attends continuing education programs as necessary

Qualifications
Qualifications:
- Must be either a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) with license to practice in Texas. Bachelor's degree in Nursing preferred
- Must have clinical data management experience with QIPP Metrics with proven quality outcomes
- Experience in MDS within the LTC industry influencing clinical and quality outcomes
- Ability to drive the Quality Measures metric offering strong outcomes that will influence the 5 Star rating
- Capability to provide education on the QIPP program as well as speak to senior level management regarding QIPP metrics
- Directs efforts at the clinic level to ensure QMs and Performance Improvement methodologies are successfully implemented and maintained
- Ensures quality of care and safe clinical practices are maintained and promoted by clinical team
- Experience with Simple LTC and Team TSI
- Knowledge of applicable regulatory, compliance, and accreditation standards and requirements
- Proven track record of compiling and analyzing large data sets
- Self-directed with organizational and priority setting skills
- Ability to establish effective communication with managers across technical and non-technical departments to identify key tasks and due dates
- Must possess high-level interpersonal communication skills to interact effectively with all levels of personnel within the organization
- Expert attention to detail
- Ability to effect improvement and change to meet QIPP metrics
- Able to travel about 30% of the time
