Form of workJOB PURPOSE JVM is hiring a Registered Nurse Case Manager to perform a full range of professional nursing services and practices on site, using government furnished facilities, equipment and supplies to a variety of patients. DUTIES AND RESPONSIBILITES Serves as a nurse case manager and patient advocate, providing advanced practice clinical, administrative, and organizational skills in managing the continuity, access, and provision of care services for complex and defined patients and address the needs of individuals and families with behavioral health needs. Will work with other providers to support case management services to high risk patients, work within a multidisciplinary team to provide patient advocacy/support, and will occasionally be required to support and travel to areas outside of their normal duty location as well as provide tele-case management services throughout the entire U.S.
Coast Guard. Functions as a member of a multidisciplinary team to meet individual and family comprehensive health needs. Duties include planning, coordinating, facilitating, and evaluating health care, serving as an advocate for patient options and services.
Utilizes communication and available resources to promote quality and cost-effective outcomes, across the continuum of care. Responsible for identification and management of patients with targeted, high risk and high cost diagnoses. Independently performs difficult and complex assessment, review, coordination, planning, monitoring, evaluation and analysis of cases.
Screens potential case management clients, following identification for appropriateness/benefit of case management services. Interviews and provides individual counseling, as needed. Addresses services needed to optimize current health, psychosocial, spiritual status, and benefits of case management services.
As required, enters approved metrics, patient data, and clinical progress notes into a prescribed database or record. Conducts comprehensive assessment of patients, to establish in detail and specificity, the nature of their care needs, as well as causal and contributing conditions and circumstances. Conducts comprehensive clinical interviews with the patient and other family members, as warranted, and collects pertinent date from all involved health care agencies, medical providers and resource programs.
Participates in a multi-disciplinary team to develop a treatment plan to address all identified conditions and problems. Identifies psychosocial aspects of anticipated care needs, to identify potential barriers to optimal health, resource utilization, and methods for minimizing such barriers. Assists the patient and the family in developing, documenting, and implementing appropriate care plans, and accessing agencies and care providers.
Applies a holistic approach to address health care needs, in collaboration with all health team members, to include the patient and the patient's family, as warranted. Operates within the established Standards of Nursing Practice, Scope of Practice, and the physical, mental, and emotional needs of the patient as personally assessed. Complies with case management standards of practice.
Interfaces/coordinates with care partners (DoD, VA, civilian, etc.) regarding patient conditions, treatment and transition of care as needed. Communicates, updates, reports and coordinates with the HSWL-SC RN Field Technical Manager (or designee as the HSWL SC) regarding all complex cases in accordance with Coast Guard policies and directives on a routine basis. Participates in the Health Services Quality Improvement Program.
Introduces innovative nursing techniques, practices, and approaches in collaboration with health care providers, to identify, plan, assess, and coordinate care programs, designed to provide efficient, comprehensive, and cost effective service for case-managed patients, through the continuum of care. Provides training and instruction to Health Services personnel and/or commands in areas where professional expertise or designation is held. Identifies problems with health care access and utilization, in both the military and civilian sectors, and recommends alternatives to overcome difficulties.
Communicates with the HSWL SC to resolve identified problems or concerns. Performs other duties as assigned. QUALIFICATIONS The RN shall possess and maintain for the duration of this agreement: A diploma, certificate or degree in health care management, quality improvement/quality assurance, nursing or other related discipline.
A diploma or degree from a National Council of State Boards of Nursing (NCSBN) accredited school of Nursing; A current unrestricted active license to practice as a Registered Nurse from one of the 50 states, District of Columbia, Puerto Rico, Guam, or the U. S. Virgin Islands; Certification for professional case management through a nationally recognized certifying entity (i.e.
Commission for Case Manager Certification, American Case Management Association, or American Nurses Credentialing Center). Maintain current certification in Basic Cardiac Life Support (BLS) for the Health Care Provider by the American Heart Association (AHA), American Red Cross (ARC), American Safety and Health Institute (ASHI), or the American College of Emergency Physicians (ACEP). A minimum of five (5) years of professional experience as an RN; with at least three (3) years of experience in the past five (5) years as a nurse case manager.
