This role requires work in the homes of clients providing Mental Health Skill Building for adults 18+, Intensive In-home for children under 18, and Crisis stabilization and Mobile Crisis services for individuals of all ages and includes flexible schedules and on-call crisis intervention. The QMHP is supervised by a Clinical Supervisor and corroborates with other staff members to ensure qualitative services. A Qualified Mental Health Professional is expected to provide face-to-face services with the client and potentially other family members primarily within the home, although they may occur in various community settings. The QMHP is required to assist in the development and implementation of the person-centered treatment plan.

Duties and Responsibilities:

• Proper and timely documentation of all information
• Involvement in the preparation of individualized service plans and updates.
• Provide various behavioral interventions in home of youth as needed.
• Model and teach problem solving techniques.
• Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include, but are not limited to social workers, psychiatrists, court officials and police, case workers and other human service professionals.
• Respond to crisis(es) on a 24 hours, seven days a week basis
• Provide linkage to community based services; provides advocacy and support for families with these services.
• Provide education around life skills and independence
• Maintain client file for compliance
• Attend meetings, training and conferences as required.
