The Quality Assurance/ Quality Management Nurse (QA/QM Nurse) plays a critical role in ensuring the delivery of high-quality patient care within the healthcare facility. The QA/QM Nurse is responsible for assessing, monitoring, and improving the quality and safety of patient care through compliance with regulatory standards, performance evaluation, risk management, and data analysis. Demonstrates an ongoing commitment to the application of best practice evidence into the clinical setting, promoting patient safety, enhancing clinical outcomes, and maintaining regulatory compliance. The QA/QM Nurse will work at our corporate office in Crown Point, Indiana when not fulfilling position-related travel obligations.
Responsibilities:
- Implement and maintain a comprehensive Quality Management program to enhance patient safety, clinical outcomes, and overall quality of care.
- Develop and implement quality improvement initiatives, policies, and procedures to enhance patient care and safety.
- Conduct audits and reviews of healthcare processes, clinical documentation, and patient records to assess compliance with standards and regulations.
- Collect, analyze, and interpret data related to patient outcomes, adverse events, and quality indicators to identify trends, areas for improvement, and opportunities for intervention.
- Collaborate with interdisciplinary teams to develop and implement evidence-based practices and quality improvement initiatives.
- Facilitate root cause analysis and corrective action planning for adverse events, near misses, and quality-related issues to prevent recurrence and promote continuous improvement.
- Perform and report the gaps between processes, procedures, and system functionality between Current State and Desired Future State Analysis
- Participate as a consultant in the implementation of chart review tools, standards of care, and outcome measures.
- Assures that all medical records are in compliance with State and Federal regulations
- Serve as a reliable source of information on the latest evidence
- Support staff development with educational activities including remediation, certification, mentoring, regulatory requirements, and EMR updates
- Participate in performance improvement committees, task forces, and other initiatives aimed at enhancing the quality, safety, and efficiency of patient care delivery.
- Evaluate the performance of healthcare providers and staff members through audits, observations, and performance metrics.
- Provide feedback, coaching, and training to improve the skills, competencies, and performance of the healthcare team.
- Identify potential risks to patient safety, clinical operations, and organizational reputation.
- Develop and implement risk mitigation strategies, protocols, and preventative measures to minimize the occurrence of adverse events and improve patient outcomes.
- Must have reliable transportation.
- Services provided by the QA/QM Nurse shall be performed both in the field and at the Company's corporate office.
Qualifications:
- Bachelor of Science in Nursing (BSN) required
- Must possess current valid license as a registered nurse in the state of practice
- Certification in Healthcare Quality (CPHQ) is preferred.
- 3+ years work related experience in the healthcare industry
- Minimum of 3 years clinical practice experience performing medical record review
- 3-5 years' experience working as a registered nurse in a clinical setting
- Experience demonstrating ability to work well with minimal supervision and on a team
- Experience reading and interpreting medical records, including handwritten records
- High level of accuracy and attention to detail
- Self-motivated and comfortable working under tight deadlines.
- Conscientious problem solver, willing to learn, takes personal pride in their work performance/accuracy
- Proven organizational and time management skills required
- Computer literacy and demonstrated proficiency is preferred in order to navigate through internal/external computer systems including Word and Excel
- Strong oral and written communication skills
- Exceptional interpersonal skills
- Abletopassbackgroundtesting,includingmotorvehiclerecordreviewanddrugscreening.
- Valid driver's license
- Active auto insurance policy
