Position Summary:

 

The Program Specialist administers the Member Passport program providing exceptional level of assistance via phone and email to senior members engaged with the program as well as performing related administrative tasks. The position also handles stakeholder inquiries for all educational and health promotion programs, and connects members with clinical health educators and other internal resources. The Program Specialist possesses exceptional interpersonal and conversational skills, be detail-oriented, and demonstrates the ability to effectively prioritize and manage multiple tasks in a fast-paced environment. The successful candidate will contribute to the overall success of the department by helping senior members participate in health education programs and preventive services to achieve their health and wellness goals.

 

Essential Duties and Responsibilities include the following:

  • Responds to member inquiries about Member Passport Program including assistance with signing up, providing status of gift card incentives, and offering program resources to help members complete preventive health and program activities.
  • Coordinates fulfillment and distribution of Member Passport incentives (gift cards) for members completing health activities; uses online vendor platform; verifies activities completed, and ensures incentives are delivered in timely fashion and correct format.
  • Responds to incoming calls and email from members interested in participating in other department education, wellness and preventive programs (flu shot clinic, diabetes/chronic disease management, movies, Bariatric Program, other developing programs). During major events, may need to answer phone calls later in the day, having to shift work schedule as needed.
  • Maintains exceptional records of all inbound and outbound communications and program activity; helps evaluate effectiveness of programs, compiles reports and provides to management upon request. Handles all member and program information in a HIPAA compliant manner
  • Utilizes exceptional communication and motivational skills for member outreach to effectively schedule 1:1 counseling sessions with health educators and registered dietitians.
  •  
  • Coordinates provider and program schedules; ensure that EMR provider and program templates and schedules are maintained; utilizes other scheduling platforms as required.
  • Manage incoming clinical health education referrals, assigning to appropriate staff and follows up as needed.
  • Provides member reminder and follow-up communications and sends educational materials as requested by staff.
  • Coordinates class/event presentations including class materials, packet preparation and relevant program materials and logistics for virtual and in-person programs.
  • Screens incoming program requests and offers member comprehensive overviews of appropriate programs. 
  • Maintains liaison with other marketing teams, departments and provider offices across the organization to facilitate collaboration and to accomplish program objectives.
  • Listens and communicates effectively with empathy and courtesy with beneficiaries/members, volunteers and other community members as well as co-workers and external contacts.
  • Coordinates priority of work with manager to successfully maintain current programs and support growth of new programs; makes recommendations for solutions to challenges identified.
  • Demonstrates clarity and accuracy in written communications and exceptional organizational skills.
  • Adheres to established work schedules and practices effective time management.
  • Ensures compliance with regulatory agencies as required.
  • Performs other duties as assigned by supervisor.

The pay range for this position at commencement of employment is expected to be between  $24 - $26 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

 

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements

Education and / or Experience:

  • Bachelor’s degree preferred
  • 2-3 years of health education or healthcare experience required.  
  • Bi-lingual (Spanish) required.
  • Demonstrates strong interpersonal and communication skills.
  • History of handling high volume of phone calls.
  • Computer proficiency in use of Word, PowerPoint, Excel and Internet; social media experience a plus.
