Major Responsibilities / Duties / Critical Tasks:
- Delivers education and patient navigation services for women in the target counties.
- Assists with the day-to-day coordination of outreach and patient navigation activities including personnel, facilities, supplies in support of project desired outcomes and timelines.
- Assists with development of program timelines and monitor progress towards meeting program milestones, metrics, and deliverables.
- Supports a cohesive team effort to accomplish approved goals, milestones, and metrics.
- Closely work with clinical partners and other third parties involved in billing.
- Monitors scheduling, billing, and invoicing of eligible grant beneficiaries on EPIC.
- Assists with the coordination of proper and appropriate training to project staff.
- Assists with establishing new partnerships and collaborations with local organizations, hospitals, and FQHC to promote access to healthcare education, screening, and treatment.
- Maintains working relationships with internal and external stakeholders and community partners.
- Assures accuracy and completeness of data on outreach/in reach efforts, patient navigation services, and screening exams for both breast and Cervical Cancer projects and compiles periodic report.
- Prepares purchasing requests for items needed for outreach activities.
- Assists in developing marketing materials and social media messages tailored to meet the unique circumstances of target populations.
Maintains confidential records /sensitive patient information with discretion and adherence to HIPAA rules.
Promotes the UT Tyler HSC mission, vision, and values by action.

Required Education / Experience:
Bachelor's degree in public health, community health, health education or related field required.
Five (5) years of relevant experience in grant funded projects. Experience working with diverse population groups preferred.
A combination of education and experience may be accepted.
Working Environment:
This is an administrative/clerical environment. Full time in-person position that involves a considerable amount of local travel within the target counties and work on weekends and after hours. The position requires travel to several counties in Northeast Texas (including Anderson, Henderson, Cherokee, Gregg, Rusk, Smith, and Van Zandt) to educate the community and engage stakeholders for at least a third of the time. The position will also involve working after hours and in weekends as needed to attend community-based events such as health fairs.
