Summary
The Coordinated Entry System (CES) Problem Solving intervention is a short-term housing solution with the goal of assisting participants in maintaining their current housing, or identifying an immediate and safe housing alternative within their own network of family, friends and social supports. This program also aims to assist households lacking alternative housing options by quickly connecting them to existing emergency or crisis housing services in order to ensure their immediate health and safety needs are met. The Program Manager of Problem Solving will lead a team of CES Problem Solving/Prevention Specialists and Patient Problem Solving Specialists in resolving clients' housing crises, that would otherwise lead to a loss of housing, by providing services including, but not limited to, short term financial assistance, housing-related support services, legal assistance and client care plans, with the goal of achieving housing stabilization.
Job Description Essential Duties and Responsibilities:
1. Provide supervisorial support and oversight for team of Problem Solving Specialists related to caseloads, scheduling, data integrity and contract goals or program changes.
2. Ability to work in a fast paced environment and successfully work well under pressure. Must be flexible and able to successfully prioritize and multi-task.
3. Daily coordination and oversight of program operations and activities.
4. Assist in recruitment, selection, training, supervision, and evaluation of direct service staff.
5. Provide individual and team supervision, support, and performance evaluations for assigned staff.
6. Assess housing barriers of participants who are unstably housed and at-risk of homelessness to determine housing and service needs, by use of assessment tools.
7. Triage into appropriate pathway depending on identified housing and service need.
8. Develop housing procurement, financial, and self-sufficiency case management plan with participants. This will include an intake interview/assessment to determine household needs, goals, and eligibility.
9. Determine eligibility and appropriate level of assistance based on client situation and ongoing monitoring of program spending and budget. Communicate team needs, updates and maintain the tracking of reports as requested by supervisor to ensure project deliverables are being met.
10. Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. client, landlord, referral source, collaborating agencies, and staff).
11. Apply knowledge of residential lease contracts to educate participants of their tenant rights and responsibilities.
12. Provide supportive services and program linkages as necessary to ensure client is able to maintain housing upon program exit.
13. Documents all services identified and provided, referrals made, and transportation provided.
14. Advocates for and actively assists individuals in obtaining services (e.g. benefits, mental health, substance abuse, housing referrals, financial assistance, home based services, training, medical services, mentoring and socialization).
15. Travels and makes home visits as well as works an adjusted work schedule including evenings and weekends as necessary.
16. Ensures data is being entered and tracked in an accurate, timely manner. This means keeping data in HMIS, charts and productivity trackers up to date.
17. Perform other duties as assigned.
Job Description Qualifications:
1. Bachelor's degree in related field (Social Work, Community Organizing, or other Human Services) and one year full-time experience working with the homeless, preferably those living with mental illness and / or substance addictions; or equivalent combination of education and experience
2. Knowledge of homeless social service providers and Coordinated Entry System
3. Able to communicate and interact with subcontractors, housing providers and community partners in a culturally competent manner
4. Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
5. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment
Job Description Preferred Qualifications:
1. Bachelor's degree in related field (Social Work, Community Organizing, or other Human Services) and one years full-time experience working with the homeless, preferably those living with mental illness and / or substance addictions; or an equivalent combination of education and experience
2. Bilingual (English/Spanish)
3. Knowledge of homeless social service providers and Coordinated Entry System
4. Able to communicate and interact with subcontractors, housing providers and community partners in a culturally competent manner
Job Description Work Environment
1. Combination of field and office environment
2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
3. On occasion walk or drive to different local sites
4. Will necessitate working in busy and loud environments
5. Will be exposed to elements like cold, heat, dust, noise and odor
6. May need to bend, stoop, twist, and sit throughout the day
