San Francisco AIDS Foundation (SFAF) envisions a future where health justice is achieved for all people living with or at risk for HIV. Position Overview:

The Benefits Coordinator and Linkage Navigator is a resource to support persons seeking access to various HIV related health services, including linkage to HIV treatment, support and prevention. This position will support people in understanding and applying for public and private benefits, accessing medical care and medications, and maintaining benefits already received. The role works with people living with HIV to facilitate access to HIV primary care and other support services. This position will be an integral part of the sexual health team. All Benefits Coordinators serve as HIV testing counselors in the clinic, Covered CA Certified Enrollment Counselors, Notary Publics, AIDS Drug Assistance Program (ADAP), and PrEP Assistance Program (PrEP-AP).


Essential Duties and Responsibilities:
 
  • Benefit Support: Provide counseling and advocacy to clients in need of assistance accessing PrEP, nPEP, Interim ART, RAPID, and comprehensive medical care. Coordinate client participation in benefits programs such as Patient Assistance Programs, Medi-Cal, Medicare, Healthy San Francisco, Covered CA, ADAP/PrEP-AP and other insurance programs.
  • Linkage Support: Coordinate with external agencies to find medical homes for individuals living with HIV for long term care. At times, the navigation process will resemble case management to address external barriers and solutions and counseling/coaching to addres internal barriers and solutions.   
  • Documentation: Maintain concise documentation and service notes according to departmental standards, including chart notes, access timelines, linkage confirmations, and refill timelines.
  • Adherence and Access Counseling: Conduct phone and email based follow up and referrals to ensure access to appointments and medication.
  • Collaboration: Respond to referrals from SFAF programs and other external agencies by conducting an intake to identify the barriers that obstruct the client in maintaining effective engagement with their medical care and treatment. Work with the nursing team to ensure proper treatment and follow up.   
  • Education and Outreach: Serve as a community leader in PrEP / HIV advocacy and education through community talks, workshop facilitation, boot camp trainings, shadowing partner sites, and outreach to target groups.
  • HIV/HCV/STI Testing Counseling: Provide client centered HIV/HCV/STI counseling services.
Essential Qualifications:
 
  • Knowledge of HIV, HCV and STI prevention, transmission and epidemiology.
  • Ability to discuss HIV, HCV, and STI risk and harm reduction strategies.
  • Ability to discuss sex and sexuality comfortably.
  • Working knowledge of the harm reduction model.
  • Knowledge of specialized benefits, such as COBRA, Medi-Cal, Medicare Parts A, B and D, and private medical insurance.
  • Knowledge of the San Francisco Department of Public Health Community Health Network and HIPAA compliance.

Physical Requirements and Work Environments:

Skill in operating equipment such as a personal computer, copy machine and telephone system.  Ability to perform routine bending, stooping, twisting, and reaching. Education and Experience
 
  • Highly Regarded: Bachelor’s/Associate’s degree (preferably in a field relating to health education, health sciences, sexuality, social sciences, etc.) with 3 years of relevant experience; or GED or high school diploma with 5 years of relevant experience. Relevant experience in both cases may include, without limitation, providing services in either a work or volunteer capacity to communities of color, LGBTQ communities, MSM populations, persons living with disabilities, queer youth, low-income individuals, people who use substances, and/or other marginalized populations. Experience need not have been on a full-time basis.
  • Highly Regarded: Current California Certification as an HIV Counselor (can be obtained within 6 months of hire as part of onboarding process).
  • Work or volunteer experience in health education, sexual health, health insurance navigation, and/or PrEP preferred. 
  • Highly Regarded: Bilingual English with Spanish, Portuguese (Brazilian), Cantonese and/or ASL preferred.

This position will have access to protected health information (PHI) and may only access, use or disclose the minimum information necessary to perform their designated role on behalf of San Francisco AIDS Foundation, regardless of the extent of access provided.

Work Environment:

This position is primarily located at 470 Castro Street, San Francisco, however it may occasionally require traveling to other office locations or off-site meetings as required by the job. Role may include evening and weekend coverage.
