The Post Acute Care Management Coordinator job focus is on assisting the Care Management staff and their patients with Social Work, Case Management, Discharge Planning and Admissions. The Post Acute CMAA is responsible for assisting with performing care coordination, resources linkage, program development, utilization review, clerical work, discharge planning, and admission for Cottage Rehabilitation Hospital patients. The Post Acute CMAA's activities will result in quality outcomes, optimal care/cost management of services and/or procedures, and high level of customer satisfaction, and contribution to an overall value-oriented experience of stakeholders and persons served. The Post Acute CMAA provides guidance and assistance with patients in need of charity, benefits, and Medi-Cal related needs.
This role requires a Bachelors, Social Work. Masters, Social Work is preferred.
This role requires 6 months experience in patient care. One to two years of patient care or case management experience is preferred.
