Position: Medicare Part B home Physical Therapist in Brooklyn

Job Description: As a Physical Therapist at Matter Rehabilitation, you will have the unique opportunity to provide exceptional one on one care and rehabilitation services to patients in the comfort of their own homes. You will assess patients' needs, develop personalized treatment plans, and deliver evidence-based interventions to promote recovery, mobility, and overall wellness. Your responsibilities will include:

• Conducting comprehensive initial assessments to evaluate patients' conditions, functional abilities, and goals.
• Developing individualized treatment plans tailored to each patient's specific needs and goals.
• Providing hands-on therapy, exercises, and therapeutic modalities to improve strength, flexibility, balance, and coordination.
• Monitoring progress, modifying treatment plans as necessary, and educating patients and their families on home exercises and self-care techniques.
• Collaborating with interdisciplinary teams, including physicians, nurses, and caregivers, to ensure comprehensive and coordinated care.
• Maintaining accurate and timely documentation of patient evaluations, progress notes, and treatment outcomes.
• Managing time efficiently to schedule and manage clients in a timely manner

Qualifications: To succeed in this role, you should possess the following qualifications:

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist.
• Proven experience in providing home-based Medicare Part B physical therapy services.
• Strong clinical skills and knowledge of evidence-based practices in physical therapy.
• Excellent communication and interpersonal skills to establish rapport with patients and effectively collaborate with the healthcare team.
• Ability to work independently, manage time effectively, and prioritize patient care.
• Compassion, empathy, and a genuine desire to improve patients' quality of life.

Pay: $70.00 - $75.00 per hour
