Job Description
We are looking for an enthusiastic and well-trained Nurse Practitioner / PA to join our team in the northern New Jersey area. The successful applicant will join our multidisciplinary team of dedicated treatment specialists to assist with managing patients in either outpatient, inpatient, or our skilled nursing facility team.
The ideal candidate will be an excellent communicator who is willing to contribute to the development of patient-centered care. They will have the ability to work collaboratively with the team to provide detailed assessments, diagnostics, and treatment plans to patients.
MUST HAVE NEUROLOGY BACKGROUND.
If full-time employment doesn't suit, we are also hiring part-time providers!
Responsibilities:
Develop and implement individualized patient treatment plans based on evidence-based best practice
Monitor and evaluate patient conditions through periodic physical examinations
Analyze test results, diagnose palliative and/or neurological problems and make appropriate referrals
Educate patients and families about pain/palliative, neurological conditions, treatment options and preventive measures
Document patient care services in medical records
Communicate with other departments to coordinate and provide care
Collaborate with multidisciplinary team to ensure holistic care and support for patients
Actively own territory of settings and build clinical relationships with key staff members including nursing, admins, physicians, aids, etc.
Qualifications:
Graduate of an accredited Nurse Practitioner / PA Program
Must be licensed by the State Board of Nursing (New Jersey)
Current certification in neurology (REQUIRED)
