Job Summary:
We are seeking a highly skilled and dedicated Pharmacist to join our team. As a Pharmacist, you will be responsible for ensuring the safe and effective use of medications by providing pharmaceutical care to patients. Your expertise in physiology, anatomy, and medication administration will be crucial in delivering optimal patient outcomes.
Responsibilities:
- Review and interpret prescriptions to ensure accuracy and appropriateness of medication therapy
- Dispense medications accurately and provide clear instructions to patients on proper usage and potential side effects
- Collaborate with healthcare professionals to develop individualized treatment plans for patients
- Monitor patient responses to medication therapy and make necessary adjustments to optimize outcomes
- Provide drug information and counseling to patients, healthcare providers, and other members of the healthcare team
- Maintain accurate patient records and ensure compliance with all legal requirements
- Participate in quality improvement initiatives to enhance patient safety and medication management processes
Requirements:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy
- Active pharmacist license in the state of practice
- Completion of a pharmacy residency program preferred
- Strong knowledge of physiology, anatomy, and pharmacology
- Proficiency in aseptic technique for sterile compounding
- Experience in acute care settings such as ICU, ER, or other acute care units
- Excellent communication and interpersonal skills
- Ability to assess patient needs and make appropriate clinical decisions
- Strong attention to detail and ability to prioritize tasks effectively
