St. Luke’s Health System in Boise is seeking a Medication Reconciliation Pharmacist to join our Retail Pharmacy Administration Team.
This pharmacist will work a variable schedule working primarily weekdays with an occasional weekend.

The Medication Reconciliation pharmacist is responsible for helping support the medication historian program across the system. Duties include:
- Reviewing medication lists that have been updated by the medication historian technician (MHT) for patients coming in for surgery
- Helping support MHTs across the system with complicated medication lists and educational seminars
- Providing training on how to gather and review medication histories to pharmacists and MHTs
- Performing routine audits of MHTs work
- Completing the annual leapfrog survey for Medication Reconciliation at the Boise, Meridian, Nampa, and Magic Valley Medical Centers
- Performing other duties and responsibilities as assigned
- Degree from an accredited pharmacy program
- Valid Idaho Pharmacist License
