Responsibilities
  • Modifies medication dosages as determined by age and weight, renal function
  • Makes appropriate evidence-based, patient-centered medication recommendations
  • Correctly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errors
  • Facilitates pharmaceutical care services and medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Effectively and proactively interacts with medical staff regarding drug therapy issue resolutions
  • Maintains accountability with drug use, including controlled drug inventory and management of hazardous waste
  • Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care
  • Participates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
  • Performs medication reconcilliation as needed
  • Participates in pharmacy operations and medication dispensing as needed
  • May serve as preceptor for pharmacy students and/or residents.
  • Supervises the care provided by licensed personnel, in accordance with the current Idaho Pharmacy Practice Act

Qualifications
  • Graduate of an accredited School of Pharmacy
  • Maintains current pharmacist license in Idaho
  • Completion of PGY-1 pharmacy residency
  • 3 years Clinical pharmacy experience in a hospital setting (required if no residency)
  • Pharmacotherapy Specialty Certification (BCPS certification) or plans to work toward certification
  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy
  • Possess computer skills for order entry, word processing, clinical interaction
  • Effective communication skills with allied health care practitioners
  • Experience in sterile products preparation
