Responsibilities:

 

The Patient Logistics Coordinator (PLC) functions as a facilitator of throughput by expediting
appropriate placement of patients within Dignity Health Facilities (St. Joseph's Hospital and Medical
Center and St. Josephs Westgate). This position incumbent's effective communication and
interpersonal skills to assist with the process of efficiently and effectively coordinating movement of
patients throughout the designated Dignity Health facilities. In addition to facilitating and managing
internal facility throughput; the PLC functions as the liaison for inter-facility transfers for direct
admissions when patient requests are placed to transfer patients to Dignity Health St. Joseph's Hospital
and Medical Center and St. Josephs Westgate. The PLC expedites incoming calls, identifies level of
hospital service needed, contacts physician or on-call physician, and connects sending physician to
accepting/admitting physician. The PLC facilitates inpatient to inpatient and ED to inpatient transfers.
The PLC follows internal guidelines to expedite the above functions.

 

• Works independently to facilitate inter facility and intra facility transfers and direct admissions, as well as the placement of
  patients as directed by the physician, utilizing the House Manager as a resource and/or Patient Logistics Manager
• Identifies potential problems and brings them to the attention of the department manager, house manager, or leadership
  team to prevent and/or resolve barriers to timely placement and/or transfer of patients.
• Responsible for obtaining surgery schedules: BNI OR, Gen OR, CVOR, Specials, Cath Lab, Litho and Endo. Reviewing
  the schedules and other admitting departments (Direct Admits, ED, and Transfer Center) and formulating a plan with the
  House Managers to admit patients into the appropriate beds.
• Must be familiar with and effectively utilizes the ADT, Throughput, and EHR and scanner software systems
• Able to calculate hospital midnight census and send to the appropriate leadership team members as required
• Obtains information from sending facility to facilitate a physician to physician telephone conversation via recorded line
  once the hospital service has been identified.
• Utilizes census and other information to maximize timely patient bed placement.
• Follow-up with sending/requesting facilities and/or physicians and document in the Transfer Center software system.
• Knowledgeable and compliant with state and governmental regulations (EMTALA, HIPPA, etc) related to compliance
  and safeguarding protected health information (PHI

Qualifications:Minimum

• High School Diploma/GED 

• One year previous experience in a health care setting

• N/A

• Basic Typing Skills Basic Computer Skills with knowledge of Word Application

Preferred

• Medical Assistant Certification

• Previous experience with high volume call setting and multitasking

• One year previous year experience in a call center or front office scheduling
