Founded in 1917, Robert Wood Johnson University Hospital Rahway is an acute care community hospital that provides inpatient and outpatient medical and surgical services. RWJ Rahway’s emergency department cares for more than 32,000 patients per year. The hospital is a Primary Stroke Center with telemedicine capability for 24/7 coverage.
The campus is home to Care Connection, a 24abed licensed subacute rehab unit owned by Alaris Health, and Kindred Hospital New Jersey a Rahway, a 34abed longaterm acute care hospital for medically complex patients who need intense specialized treatment for an extended period of time. The hospital provides extensive therapeutic services including physical, occupational speech and hearing. For those with heart and lung disease, the hospital has cardiac and pulmonary rehabilitation.
It has a peripheral neuropathy clinic, as well as diabetes counseling. Physical and aquatic therapy centers are located in its fitness centers in Scotch Plains and Carteret. It has a joint replacement center that consistently receives praise from patients for its expertise.
The hospital has extensive outpatient diagnostic and lab services. The hospital now has a surgical weightaloss program that is a Center for Excellence. Its Center for Wound Healing and Hyperbaric Medicine has an average “days to heal rate” significantly below the national average. It has a sleep center and balance center. The hospital’s community education program provides education, screenings, support groups, healthy cooking, and healtharelated demonstrations at its fitness centers and in the community.
Outpatient Services include wound care and hyperbaric medicine; pulmonary rehab; cardiac rehab; physical, occupational and speech therapy for adults and children; incontinence treatment; diabetes management and weightaloss for adults and children; neuropathy treatment; balance testing; and sleep center.
The Patient Care Coordinator assists the Nurse Manager with ensuring that quality nursing care is delivered to all patients and his/her nursing units in accordance with the philosophy, policies, procedures and program plans of the hospital. Assists with the development of patient care standards, orientation and continuing education. Provides input into evaluation of staff performance in accordance with organization policies. Participates in nursing Quality Improvement. Assists with unit administration, including scheduling, payroll and maintenance of supplies and equipment. Maintains and role models clinical skills necessary to provide care to patients.
QUALIFICATIONS:
Graduate of an accredited school of nursing and currently licensed to practice as a Registered Nurse in New Jersey.
Bachelor of Science in nursing preferred.
