We are looking for a Coordinator to join our team!
The Coordinator is responsible for scheduling, monitoring call in, documenting as necessary, and ensuring case coverage for all cases. The ideal candidate will have experience with the following:
Responsibilities:
- Answer all incoming phones calls. Tries as best as possible not to miss any incoming phone calls and places people on hold as necessary as calls come in.
- Makes all schedule changes and updates as necessary and makes sure to cover all open cases.
- In the event that there is a call out, the coordinator is required to contact the patient, ask if the patient needs replacement, and makes every effort to cover the case.
- Tries to maximize visits.
- Monitors Call Dashboard as according to agency policy, and documents in every visit each time an aide fails to call in or out for a visit.
- Makes sure to notify patient and vendor of any change in service in a timely manner.
- Documents all miss-visits with the appropriate reason.
- Places any aide which requires discipline, on the discipline smart sheet so that the appropriate discipline can be done.
- Places all incidents and change on Incident smart sheet so an incident report can be completed.
- Documents all changes in patient’s status to interim order smart sheet (increases, decreases, hold, hospitalizations, discharges, resume services).
- Transfers all patient’s complaints to the Administrator.
- Send daily reports as directed.
- Assist with all agency job functions as needed due to absence and agency overflow.
- · Responsible to complete all tasks before the end of the workday.
- Other duties as assigned.
Qualifications
- Previous coordination or staffing experience is preferred.
- Basic computer skills required. The ability to utilize basic computer software integral to agency operations after initial training period.
- Must have the ability to write and document appropriately.
- Must have phone skills and proper communication.
- Strong multi-tasking capability.
- Be a self-started and take responsibility.
- Be aware of all policies and procedures related to coordination (given upon hire).
Job Type: Full-time
Pay: From $19.00 per hour
