Patient Care Coordinator - The Woodlands

We are seeking a full time, Patient Care Coordinator who will be responsible for initial patient contact following the physician consultation.
ESSENTIAL RESPONSIBILITIES:

• Review patient chart and physician notes; evaluate the patients plan for care.
• Contact patient seven (7) days post physician consultation to introduce yourself and provide a warm welcome to Aspire HFI.
• Educate patients about the process from testing to treatment.
• Facilitate patient connection with any/all staff members for questions or scheduling.
• Data collection of all patient communication.
• Improve patient experience and retention while reducing patient cancellation throughout their infertility journey at Aspire HFI.
• Ensure optimal productivity and efficiency.
• Contribute to team effort by maintaining an excellent rapport and working relationship between all Aspire HFI staff other departments within practice and the Prelude network
• Demonstrate excellent communication and interpersonal skills
• Other duties as assigned

EDUCATION AND/OR EXPERIENCE:

• High School Diploma or equivalent required
• Minimum of two years of professional experience in a healthcare setting.
• OB/GYN or Fertility experience a BIG plus!
