We are seeking a full time, Patient Care Coordinator who will be responsible for initial patient contact following the physician consultation.
ESSENTIAL RESPONSIBILITIES:
- Review patient chart and physician notes; evaluate the patients plan for care.
- Contact patient seven (7) days post physician consultation to introduce yourself and provide a warm welcome to Aspire HFI.
- Educate patients about the process from testing to treatment.
- Facilitate patient connection with any/all staff members for questions or scheduling.
- Data collection of all patient communication.
- Improve patient experience and retention while reducing patient cancellation throughout their infertility journey at Aspire HFI.
- Ensure optimal productivity and efficiency.
- Contribute to team effort by maintaining an excellent rapport and working relationship between all Aspire HFI staff other departments within practice and the Prelude network
- Demonstrate excellent communication and interpersonal skills
- Other duties as assigned

EDUCATION AND/OR EXPERIENCE:
- High School Diploma or equivalent required
- Minimum of two years of professional experience in a healthcare setting.
- OB/GYN or Fertility experience a BIG plus!
