Summary:
The NICU Patient Care Coordinator facilitates a smooth transition between the infant’s admission in the NICU and discharge to home. The NICU Patient Care Coordinator in collaboration with the NICU nursing leadership team will establish and maintain relationships with outside entities that benefit the NICU patient during their NICU stay and post-hospitalization. The NICU Patient Care Coordinator utilizes evidence-based practice, clinical expertise, problem solving and communication skills to achieve optimal outcomes within an appropriate time frame.
Job Responsibilities:
- Works in collaboration with NICU leadership to optimize the patient and family experience.
- Works with physicians, surgeons, consulting services, and hospital personnel to develop an organized care plan for patients.
- Works collaboratively with the clinical case manager, social work, outside agencies and families to coordinate care of the complex patient.
- Maintains a working relationship with community agencies that are essential to the discharge plan of care for NICU patients/families.
- Utilizes clinical information systems to promote safety and quality to align with NICU quality outcomes.
- Assesses learning needs of patients, families, and staff and provides educational strategies to meet those needs.
- Collects mandatory reporting data for the NICU population to external agencies.
- Participates in policy/procedure/protocol/pathway development, committee activities and projects as they pertain to the program development.
- Participates in and complies with ongoing certification, regulatory and accreditation readiness sustaining a working understanding of requirements and accreditation standards. Ensures areas of responsibility consistently meet these standards.

Position Requirements:
Education: Earned nursing degree at the baccalaureate level or higher. MSN preferred.
Licensure: Must be licensed and currently registered to practice as a Registered Nurse in the State of IL.
Certifications: CPR Required. Certification in Neonatal Intensive Care preferred or obtained within two years of hire.
Special Skills: Excellent interpersonal, communication, problem solving, autonomy, and decision-making skills. High level of analytical ability and the skill to work in complex clinical or hospital systems required. Experience in clinical program development and hospital operations preferred. Knowledge of Joint Commission & IDPH performance improvement standards. Ability to use computer systems including electronic medical records, chart reviews, and programs such as Excel, Word, and PowerPoint highly preferred.
Experience: Minimum 4 years Level III – Level IV NICU experience required
- Ability to travel throughout the Medical Center.
