Responsibilities
As a member of a multi-disciplinary outpatient Palliative Care team, this job is responsible for assessing and providing support/intervention for the social and emotional needs of patients and their families who are living with and navigating life-limiting serious illness. Work involves professional activities such as specialty-level Palliative Care consultations which include goals of care discussions, psychosocial assessments, and mental health evaluations. Additional duties include ongoing therapy sessions for adjustment disorder, anticipatory or complex grief, support with coping and existential distress. An incumbent will also assist with symptom management, referrals, crisis intervention, environmental assistance, patient advocacy and end of life planning.
Qualifications
-Master's degree in Social Work (MSW).
- Current Licensed Independent Clinical Social Worker (LICSW).
- Two years of related work experience (e.g. Palliative Care/hospice) that would demonstrate attainment of the requisite job knowledge/abilities is preferred.
