Compensation:
- $5,000 Sign-On Bonus
- $32.00 - $44.83 per hour
- Referral Bonuses are available for any CNA, MA, or RN you refer that gets hired at CCHS
Looking to think out of the box? Our Clinic Services teams work hard to bring the needed services to residents of this rural community. As a member of this team, your skills and advocacy for the patient will be recognized. This position allows you to work with both the Population Health and Palliative Care teams, focusing on health education for the surrounding area, identifying at-risk patients, and assisting with their care so that they may deescalate their symptoms. Healthcare is not just what someone experiences in their provider's exam room, but takes a holistic approach to address psychiatric challenges, wound care, symptoms management, and socioeconomic difficulties.
In this position, you will have the opportunity to work in a variety of settings including clinics, patient homes, and assisted living facilities. You will focus on chronic care management, Palliative Care, disease-specific programs, and Diabetic education. To succeed, you will have a confidence to work independently while also thriving in a team environment. Some travel is required for this position and is reimbursable.
