**This recruitment is designated as a hybrid role with routine telework and periodic in-person community outreach required. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other states. The work schedule is Monday-Friday from 8:00AM - 5:00PM.**

We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team as the Older Adult Clinical Services Specialist/Mental Health. In this position, you will work as a part of a multidisciplinary team across Aging, Disability and Veterans Services (ADVS), Developmental Disabilities, Addictions Services and Mental Health Services to benefit Older Adults with multiple service needs. This position will be responsible for identifying current resources in the community specific to the Older Adult population, coordinating services with other disciplines such as Health, Addictions, Developmental Disabilities and Mental Health to best meet the needs of the individual, and to provide complex care consultation and training. Ideal candidate will have a passion working at the intersection of mental health, substance abuse, and aging.
As the Older Adult Clinical Services Specialist/Mental Health, other duties include:
Coordination of Direct Services
  • Provide care coordination and collaboration between systems such as Aging, Disability and Veteran Services (ADVS), Mental Health (MH), Alcohol and Drug (A&D), Intellectual & Developmental Disabilities (IDD) Consumer groups, advocates, health and medical care and other community services.
  • Maintain partnerships with agencies, healthcare providers and service providers that work with Older Adults and people with disabilities.
  • Active elimination of system barriers for this population.
  • Consultation for transitions and discharges from Long Term Care as well as diversion and/or discharge from jails and hospitals, as needed.
  • Key role is to bridge multiple systems that serve Older Adults and people with disabilities, collaborate on projects with Tri-County (Washington, Clackamas and Multnomah) partners in these efforts.

Education
  • Develop and promote training opportunities including Mental Health First Aid for Older Adults, CALM trainings, A&D trainings, etc. to address gaps in treating the older population, best practices, and available community resources.
  • Plan training jointly across agencies and counties; provide information as needed to families, primary care providers and local community re: regional and statewide resources.
  • Participate in individual/group supervision and training to support job functions.

Identification of Resources, Documentation
  • Identify access and referral information for existing services in the community.
  • Maintain database on resources for placements and facilities including community, long-term care and acute care.
  • Identify gaps in service delivery systems, barriers to resource access.
  • Work with State-level Coordinator and other Older Adult Clinical Services Specialists to ensure client needs are appropriately and adequately addressed.
  • Identify outcome-based elements tracked, document data, quarterly reporting for OABHI (Older Adult Behavioral Health Initiative).

Required Minimum Qualifications:
Education/Experience: Must meet the standards of the Qualified Mental Health Professional - (QMHP-C) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHP and/or licensed Board of Oregon (LPC, LCSW, PHD, etc.):
  • Graduate degree in psychology or
  • Graduate degree in social work or counseling or
  • Graduate degree in a behavioral science field

Other:
  • Driver's License required
  • This position requires a background investigation, which may include being fingerprinted.
