Responsibilities:
- Conduct patient assessments to determine their health needs and goals
- Develop personalized health plans for patients based on their individual needs and goals
- Provide guidance and support to patients in implementing and maintaining healthy lifestyle changes
- Educate patients on nutrition, exercise, stress management, and other aspects of a healthy lifestyle
- Monitor and track patient progress towards their health goals
- Collaborate with other healthcare professionals to coordinate care for patients
Requirements:
- Bachelor's degree in a related field (e.g., nutrition, exercise science, public health)
- Certification as a Health Coach or similar certification is preferred
- Strong knowledge of nutrition, exercise, and other aspects of a healthy lifestyle
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
