A Chancy Drugs Health Coach is expected to enhance communication and build relationships with patients regarding their therapies and wellness. Strengthen relationships with prescribers and medical community to provide better patient care and satisfaction. Meet all expressed needs of patients and discover unexpressed needs of patients.
Major Functions:
- Ensures patient’s medication adherence by monthly communication and reporting.
- Acts as a liaison for pharmacy staff onboarding new patients to the Packaging program.
- Identifies non-adherent patients to achieve medication adherence through Medication Synchronization.
- Performs comprehensive assessments of patient’s disease states such as diabetes, hypertension, etc.
- Provides assistance in partnership with pharmacists in managing plans of care for disease states.
- Performs syncing of medications to a specific date in pharmacy software.
- Manages short filling medications to align with the patient's specific sync date.
- Adjudicates claims for fills and addresses/resolves rejections and prior authorizations.
- Manages refill requests to prescribers through pharmacy software.
