Job Summary: The person in this position will assist the Providers and the flow of the clinic with retrieving of all Medical Records and also adhere to their duties and carrying on the necessary functions required in accordance with established policies and procedures.
This position is responsible for the following:
· Retrieving charts for every scheduled Provider for the following Day. (When one Medical Records Clerk is out of the office, then the clerks will huddle and discuss even distribution of the tasks, and take over pulling of daily charts).
· Ensures that if charts cannot be located in a timely manner will then follow up with Manager.
· Timeliness when retrieving of charts upon request.
· Compiling, verifying, and filing of Medical Records
· Preparing records and maintaining records of all patients up to date.
· Reviews Medical Records for completeness
· Assembling records into standard order, and files records in designated areas according to applicable alphabetic filing system.
· Locating and delivering Medical Records requested by Providers (appointments, pulls, and/or walk in patients).
· Ensuring all charts are in designated areas such as Volume Charts, Deceased patient charts, and charts needing to be transferred to storage.
o Break down of charts for
§ Volumes – aware of protocol
§ Deceased charts – aware of protocol
§ Inactive patients (not seen in 6 months move upstairs to make room for new patient files)
§ Inactive patients (3 years or more go to storage)
· Understanding Daily order of - filing Lab/Radiology/Hospital reports and the process from assigned Provider and help others when needed.
· Act as a Liaison between staff and administration regarding chart issues and concerns
· Daily incoming mail Process:
o Use date stamp on all correspondence, labs, radiology, consultations etc.
o Delivery of all incoming mail (Should be delivered on the date stamped).
o Ensure Clerk Initials
o Once stamped all documents must be delivered to delegated Provider.
· Expectation and to be aware - Loose route slips Process:
o Keep Lead/Clinical Manager apprised of pending Routes to be filed
o Ensures communication with check out staff at all suites to ensure filing in a timely manner (End of every shift)
o Ensures all route slips need to be in a centralized area and all staff should be aware
o Ensures that the accordion and/or file should be kept in the same area at all times
· Ensures documents are ready to be filed in chart by punching holes in all documents (Stack not one at a time)
· Ensure Medical Records supplies are well stocked
· Understands the importance of helping with feedback in the Medical Records department to help facilitate a positive experience
· Understands that as a team member must help facilitate daily duties and PULLING of walk in charts
o On occasion help Provider with interpreting
· Work independently with direction from the Clinical Manager
· Other duties as assigned

Qualifications, Education, and Experience:
1) Must have High school diploma or equivalent.
2) Bilingual in English/Spanish preferred but is not required.
3) Experience as a Medical Records Clerk preferred but is not required.
4) Must have leadership abilities and demonstrate ability to work independently.
