MDS Coordinator, RN

We are looking for a full-time Registered Nurse to join our nursing team as the MDS Coordinator. Wages are competitive and based off of related experience.

Primary Responsibilities:·

Follows instructions willingly, able to meet deadlines, honesty, punctual, and dependable. Has leadership ability with evidence of skills in creativity, integrity, and communication.

· Is able to display tact and good judgment in dealing with varying situations involving physicians, patients, and their families and visitors as well as management and staff.

· Must have basic computer skills or be able to acquire such skills. Ability to type 50 words per minute is desirable.

· Must be knowledgeable of standards of care, including but not limited to: restorative care, comprehensive assessment of residents, and the development of care plans.

· Monitoring of ancillary department sections of the RAT for timely completion, including notification of the individual departments if deadlines are in danger of not being met by the departments.

· Prompt notification of DT members when a change of condition MDS/RAI is initiated

· Monitoring of input into each MDS to ensure accuracy and consistency of responses across the interdisciplinary assessments, acting as facilitator to help resolve disagreements among team members and assists in completion of RAI as needed.

· Maintaining a current knowledge base regarding state and federal regulations relative to the RAI process and PPS.

· Actively participating in the development of and final completion of an individualized, comprehensive, interdisciplinary plan of care for each resident within regulatory time frames.

· Ensuring that care plans are updated and maintained to reflect the resident's changing needs.

· Actively participate in the facility QA/QI process.

· Mentoring and teaching of nursing and ancillary staff to ensure that all team members are aware of and competent in their roles in the RA /PPS process.

· Expected to respond to emergency situations involving the safety of residents, other employees, and the facility. Includes the ability to assist with a possible evacuation of residents.

· Other duties and responsibilities assigned by the D.O.N. within the scope of the MN Nurse Practice Act.

Qualifications:

• MN License and in good standing on MN Board of Nursing
• Ability to pass a background check
• MDS experience preferred
• Must be able to analyze, review and judge different situations they face and take the right actions according to the situation
• Patience, compassion, and organizational skills are a must in our constantly evolving work environment
