MDS Coordinator

```Duties```

-This position is hourly with VERY competitive wages!!!!!! RN or LPN License Required

-The main function of this position is to assess resident needs, plan, direct, coordinate, implement, control, evaluate and improve the quality of resident care provided by the direct care staff using RAI 3.0 and other quality measures already in place.
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations
- Ensure accurate and timely completion of MDS assessments, including assessment scheduling, data collection, and submission
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Review medical records and documentation to ensure accuracy and completeness
- Conduct ongoing monitoring and auditing of MDS assessments to identify areas for improvement
- Provide education and training to staff on MDS assessment processes and requirements
- Participate in care planning meetings to ensure resident needs are accurately reflected in the MDS assessments
- Stay up-to-date with changes in regulations related to MDS assessments and implement necessary changes in processes

```Skills```
- Strong knowledge of HIPAA regulations and ability to maintain confidentiality of resident information
- Experience with managed care and utilization management systems
- Familiarity with discharge planning processes and medical management principles
- Proficiency in medical coding and understanding of coding guidelines
- Knowledge of hospice care regulations and documentation requirements
- Excellent organizational skills and attention to detail
- Strong communication and interpersonal skills to effectively collaborate with interdisciplinary team members
- Ability to analyze data, identify trends, and make recommendations for improvement
