MDS COORDINATOR

Starting Salary is based on Experience, Education and Licensure/Certifications!

• RN Registered Nurse with MDS/Care Plan Experience.
• or
• LPN with EXTENSIVE MDS/Care Plan Experience.
• Salary Based on Education, Licensure, Experience & Certifications.
• Perform assessments of residents in facility per Medicaid/Medicare guidelines, data entry of assessments, transmit assessments to Medicaid and Medicare.
• Review medical records, verify insurance coverage and eligibility for Medicare, Medicaid or 3rd party payment.
• Employ MDS coordinator, supervisor and staff RN of long term care and subacute patients.
• Perform PPD testing, regulator fitting, flu shots, and hepatitis vaccination for staff.
• Attend Alzheimer training regularly, provide assistance with daily living activities; such as bathing, grooming, and toiletry.
• Coordinate MDS assessments and care planning for SNF; work with the interdisciplinary team to develop individualize care plans for patients.
• Conduct meetings with residents, family members, and the IDT members to collect accurate data to complete a comprehensive MDS.
• Initiate & complete MDS/RAI process tracking forms and state submissions timely in software system and assist/support IDT team with computer questions.
• Maintain all employee health records to ensure physicals, immunizations, and PPD plants are timely in accordance with regulatory requirements.
• Submit all medicare/medicaid assessments to CMS.
• Produce all financial documentation for Medicare/Medicaid reimbursement according to CMS guidelines.
• Facilitate content review and regulatory updates for new trainees and tenure coordinators.
• Utilize organization skills to coordinate weekly campus-wide worship service liaising with volunteers and creating PowerPoint presentations

Job Type: Full-time

Pay: $64,402.00 - $72,802.00 per hour

Expected hours: 40 per week
