Mds Coordinator

Aspire Senior Living is seeking MDS Coordinator, experience preferred but willing to train a dedicated and enthusiastic nurse willing to grow their skill set in this rewarding career.
Job Description/Essential Functions:

• Coordination of facility's RAI assessments in accordance with state and federal regulations
• Collaborating with health plans, physicians and IDT (Interdisciplinary team) for assurance that resident meets eligibility and coverage criteria
• Auditing Medical records for the presence of supportive documentation for items coded on MDS
• Care plan completion
• Provide ongoing education to department managers, physicians and nursing staff as needed
• Accurate and timely completion of MDS assessments
• Attend weekly meetings to promote continuity of care and case management

• Insurance/PTO/Sick Time

• Active Missouri LPN or RN license in good standing (RN preferred)
• Intermediate computer knowledge including Microsoft Office, Outlook, PowerPoint, Teams or ability to learn
• Excellent written, verbal, and interpersonal skills
• 1 year experience preferred
• Familiarity with MatrixCare is a plus
