Lakeview Post Acute is excited to be adding a MDS nurse to our team. We are looking for someone who is experienced in MDS, and preferably completed the RAC-CT. This position should be highly experienced in PDPM and managed care providers reimbursement programs.
The primary purpose of this position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of the state and the policies and goals of this facility.
Responsibilities:
- Conducting Resident Assessments: Perform comprehensive assessments of residents' physical, mental, and psychosocial needs using the Minimum Data Set (MDS) tool.
- Care Planning: Develop individualized care plans based on assessment findings, in collaboration with the interdisciplinary team, including physicians, nurses, therapists, and social workers.
- Documentation: Maintain accurate and up-to-date documentation of assessment findings, care plans, and resident progress in accordance with regulatory guidelines.
- Regulatory Compliance: Ensure compliance with federal and state regulations related to resident assessments, care planning, and documentation, including requirements from agencies such as the Centers for Medicare and Medicaid Services (CMS).
- Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the quality of care provided to residents, including monitoring outcomes and identifying areas for improvement.
- Interdisciplinary Collaboration: Collaborate closely with members of the interdisciplinary team to ensure coordinated and comprehensive care for residents.
- Staff Education: Provide education and training to facility staff on MDS assessment processes, care planning principles, and regulatory requirements.
- Resource Utilization: Optimize resource utilization by identifying opportunities for efficiency and effectiveness in care delivery.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license to practice.
- Previous experience in long-term care or geriatric nursing preferred.
- In-depth knowledge of MDS assessment processes and regulatory requirements.
- Strong clinical assessment and critical thinking skills.
- Experience with PDPM
- RAC-CT preferred*
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Attention to detail and strong organizational skills.
- Proficiency in computer documentation systems.
Job Type: Full-time
Pay: $80,000.00 - $85,000.00 per year
