Position:MDS Coordinator
Salary: $140,000 - $150,000 per year
Are you passionate about providing top-notch care for residents in a skilled nursing facility? Parkview Care and Rehabilitation Center is seeking a dedicated MDS Coordinator to join our team.
Requirements:
- A current New York State licensed Registered Professional Nurse who possesses a Registered Professional Nursing degree from an accredited college school of nursing program
- Demonstrated leadership ability in nursing practice in a Long Term Care setting required
- Must possess and maintain all necessary certifications for CPR/BLS and other certifications, in-services as required by this facility
- Completion of the MDS 3.0 training experience required
- Employee must possess a working knowledge of the Medicare/OBRA assessment and regulations in a Long Term Care Facility
- Employee must possess working knowledge of computer and associated skills
- Attends in-service education as required by position title
Job Description:
- You must have knowledge of PDPM, CMI, CMS guidelines and the RAI process
- Monitor residents for clinical issues, significant changes in condition, and together with the IDT, utilize critical thinking skills to promote positive outcomes
- Have an understanding of MDS requirements for Medicare, Medicaid and Managed Care payers and be able to schedule MDS assessments as per those specifications
- Completion of the MDS, including Care Area Assessments and link appropriate Care Plans in accordance with state and federal guidelines
- Develop and update care plans
- Schedule and chair Care Plan meetings
- Coordinate with the IDT to maximize CMI reimbursement
- Initiate, maintain, and obtain MD signatures for Medicare Part A certifications
- NOMNC completion and review
