Job Summary
The Minimum Data Set (MDS) Coordinator is responsible for the completion of the MDS, a comprehensive assessment tool used to collect data on the functional status and needs of long-term care residents. The ideal candidate will have a passion for elder care, a strong attention to detail, and a commitment to providing quality care to our residents.
Responsibilities
- Complete the MDS for all residents in accordance with federal and state regulations
- Collect data from residents, their families, and other healthcare professionals
- Analyze data and develop care plans for residents
- Collaborate with other healthcare professionals to ensure that residents receive the highest quality of care
- Maintain accurate records of all MDS data
Qualifications
- Current state license as an RN or LPN
- Strong attention to detail
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
