In support of the Director of Nursing, responsible for the scheduling of MDS assessments (skilled and long-term care) per RAI guidelines to capture the most accurate level of care.
Accountability incentive of $1.50/hour and sign-on bonus of $3,000.
- Responsible for ensuring MDS assessments are scheduled and locked (Z0500B) per Resident Assessment Instrument (RAI) guidelines.
- Submit to the QIES/ASAP database per RAI guideline.
- Responsible to provide skilled charting guidelines for skilled residents.
- Responsible to track changes with the InterACT log.
- Complete the ECS compliance log per the defined ministry process and procedures.
- Complete reports as assigned by the ED, DON, and Director of MDS/Case Management.
- Provide primary diagnosis/codes for referrals.
- Ensure resident care plans are updated with Physical Order copies.
- Ensure resident care plans are updated with therapy representative (typically on a daily basis).
- Attends Clinical Management Meetings as requested by DON or Clinical Manager.
- Reports hazardous incidents, including but not limited to, injuries, equipment, etc. to the appropriate Manager/Supervisor.
- Implements and follows disaster plans in the event of an emergency.
- Participates in committees as assigned i.e. QAPI, Care Conferences, and Risk Meetings.
- Performs other job duties as assigned.
- Associate’s Degree in Health Care Administration or related field required.
- Current State of Ohio LPN or RN License preferred. 2-3 years of MDS experience required.
- Familiarity with Microsoft Office products including, but not limited to, Word, Excel, and PowerPoint. Ability to learn new software application as required by the position.
- Requires excellent oral and written communication skills, including the ability to interact with senior executives, managers, supervisors and direct care staff, as well as residents, families and physicians.
- Understands directions; communicates and responds to inquiries promptly.
