Department: Case Management- Schedule: Full-Time, Exempt
- Hospital: Ascension St Thomas
- Location: McMinnville, TN
- Manage professional and clerical associates who work in Case Management, as well as tracking of the post-acute period and provision of disease management, and contracting with internal and external resources.
- Communicate with administration and medical staff in matters related to appropriateness of hospital admissions, discharge planning, and length of stay.
- Serve as content specialist for staff in the areas of utilization criteria, appeal and review process, and Case Management system documentation.
- Develop staff schedule and revises assignments daily to assure optimal unit coverage and distribution of workload within staffing standards. Cover on units as needed and serves as a resource for complex Case Management issues.
One or more of the following required:
- Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
One or more of the following preferred:
- Case Manager credentialed from the American Case Management Association (ACMA).
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
- High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
- Master's degree preferred.
- 3 years of experience required.
- 1 year of leadership or management experience required.
