The Case Manager works directly with patients and their families in close conjunction with physician and other health care team members to implement case management services. The Case Manager also maintains contact with service providers to facilitate community resources and referrals.
ESSENTIAL DUTIES/RESPONSIBILITIES:
- Conduct intake assessments on all enrolled patients to determine case management needs.
- Provide supportive and basic counseling to patients.
- Interview patients to obtain case management information relevant to monitoring activities as outlined in the plan of care.
- Conduct follow-up with patients after case management service/intervention is completed to assure the appropriateness of service is rendered.
- Document contacts and enter into the case management record by specified timeframes after intervention.
- Coordinate and implement intervention plan in accordance with established policies and procedures. Work collaboratively with social agencies as required implementing the plan.
- Facilitate the completion of forms and assists in the provision of information necessary to provide services to patients.
- Contact and/or make referrals to outside agencies necessary to facilitate food services, transportation, housing, utility assistance, mental health, substance abuse treatment and case management for all patients.
- Prepare and maintain required reports in established formats and time frames.
- Conduct home visits to follow-up on missed appointments and assist with re-entering into care.
EDUCATION/EXPERIENCE:
- Bachelor’s degree in Social Work (BSW) required.
- Master Degree in Social Work (MSW) preferred.
- 2 years of case management experience.
- Knowledge of available HIV/AIDS programs and community resources required
