JOB TITLE: Case Management Liaison

REPORTS TO: Senior Director of Compliance and 340B/Senior Client Services Director

STATUS: Regular Full-Time, Non-Exempt

COMPENSATION RANGE: $18-$20

Note: Compensation based on factors such as job duties, education level, position autonomy and supervisory responsibilities.

POSITION SUMMARY:

The Case Management Liaison is responsible for assisting in navigating clients to Case Management and clinic services. The Case Management Liaison will assist in answering phone calls and service inquiries related to Case Management and the clinic, as well as other Choice Health Network services as needed. The Case Management Liaison is also responsible for data collection and entry and will assist with chart documentation, scanning, and other front office needs, as necessary.

RESPONSIBILITIES AND DUTIES:

  • Answers telephone in a courteous manner and takes messages and forwards calls during regular business hours (8:00 a.m. to 4:00 p.m.)
  • Coordinates appointments with Case Management and clinic, including a sufficient understanding of documentation needed for upcoming appointments.
  • Responsible for daily insurance verification, appointment reminders and missed appointment calls for clinic and Case Management appointments
  • Responsible for monthly retention in care efforts, including phone calls and re-engagement letters
  • Responsible for monthly Ryan White recertification reminder phone calls and letters
  • Completes preliminary intake for all Case Management and clinic patients, and coordinates/schedules subsequent services as needed
  • Completes all outside medical records requests for clinic patients
  • Completes 340B eligibility assessments and referrals for all new and returning patients
  • Responsible for weekly missing fills investigation and reporting for 340B
  • Responsible for assisting in providing and logging client assistance items, such as food and gas cards
  • Scan documents and assist in charting in Electronic Medical Records.
  • Complete data collection for quality assurance and improvement projects across each region served by Choice Health Network with the guidance of the Client Services Director and Medical Case Manager.
  • Complete data entry for Ryan White funded services on a weekly basis
  • Complete other various data entry tasks.
  • Other duties as assigned.

JOB QUALIFICATIONS AND SKILLS:

  • Demonstrate multi-cultural sensitivity and experience or interest in working with underserved communities.
  • Ability to multitask and prioritize projects.
  • Familiarity with Microsoft Office, Microsoft Word, Microsoft Excel, and other Microsoft programs.
  • Demonstrate flexibility and enthusiasm for the services provided by Choice Health Network
  • Demonstrated knowledge of community resources, social service agencies, and landlords.
  • Fluency in the English language is required. Spanish speaking ability is an added asset but is not required.
  • Must be able to stand for 45 minutes at a time during an eight-hour workday.
  • Must be able to walk for 30 minutes at a time during an eight-hour workday.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.
  • People with lived experience of substance use, incarceration, homelessness, and sex work; people of color, members of the LGBTQIA+ community; and people living with HIV and/or hepatitis C are encouraged to apply.

EDUCATION AND EXPERIENCE:

  • Experience with computer systems and Microsoft Office programs
  • Four-year degree in human services preferred but not required
  • Two or more years working with underserved communities preferred

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
