Role Summary
Care Coordinator will engage in an assigned case load of patients and/or participate with new patient enrollments. Via telephonically, the coordinator will focus on health care needs, care planning, appointment scheduling, communicating with nephrologist, primary care provider, and other specialists to accomplish an overall goal of improving outcomes for the nephrology patient. The coordinator will use a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services for the patient in conjunction with the nephrologist provider. The Care Coordinator will support the patient through the continuum of care and promote the highest level of quality care. Circle of Care's purpose is to assist in slowing down the progression of chronic kidney disease and to help prevent hospitalizations. This is achieved by involving the patient in their own care outside of the office visits. Circle of Care's primary focus is with patients who are at high risk of disease progression.
Responsibilities include:
- Creates, reviews, and revises the nephrology patient's plan of care. The coordinator will focus on documentation of the patient's lab values, blood pressure, blood sugar, weight, diet/nutrition, and any other health related behavior that affects outcomes.
- Establishes patient goals and interventions.
- Ensures stage appropriate referrals for education, vascular access, and transplant are completed.
- Maintains consistent communication with the patient for every patient in the assigned case load.
- Participates in patient enrollment into the Circle of Care Program by gathering all the clinical elements necessary. Together with educating the patient regarding the benefits of the program.
- Performs medication reconciliation.
- Collaborates with nephrologist and other specialists in delivering and coordinating necessary care/services.
- Acts as a patient advocate, liaison, and information resource; escalating all critical information to the nephrologist in a timely manner.
- Monitors and reports patient outcomes through a plan of care.
- Provides chronic care education for nephrology and chronic/complex conditions.
- Provides culturally appropriate education.
- Performs timely and accurate documentation.
- Perform other duties and responsibilities as required, assigned, or requested.
Qualifications:
- High School Diploma or equivalent required.
- Medical Assistant degree, diploma, or certificate with 1 year of clinic experience.
- Medical Assistant with nephrology experienced preferred.
- Requires medical records experience/exposure.
- Broad understanding of physician front- and back- office practices, preferably in nephrology.
- Knowledge of medical terminology.
- Knowledge of HIPPA regulations.
- Knowledge of insurance carriers, copays, and eligibility verification.
- Knowledge of EHR/EMR systems, AthenaNet preferred.
- Requires working knowledge of Microsoft Office Suite.
