```Duties:```
- Provide compassionate and comprehensive care to patients in hospice settings
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and report any changes or concerns to the healthcare team
- Collaborate with interdisciplinary team members to ensure holistic care for patients
- Educate patients and their families on disease management, symptom control, and end-of-life care options
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Assist with discharge planning and ensure smooth transitions between care settings
```Skills:```
- Proficient in pain management techniques and strategies
- Knowledge of medical terminology and ability to accurately document patient information
- Experience with medication administration, including dosage calculations and proper administration routes
- Familiarity with HIPAA regulations and ability to maintain patient confidentiality
- Experience working with pediatric patients and their families
- Strong understanding of discharge planning processes and resources available for post-hospice care
- Expertise in acute pain management for patients with complex medical conditions
- Ability to provide health coaching to patients and families, promoting self-care and wellness
- Experience working in an ICU or critical care setting is a plus
- Familiarity with Athenahealth or other electronic health record systems is preferred
