JOB TITLE: Social Worker
DEPARTMENT: Social Services
PRIMARY PURPOSE: To identify and meet psychosocial needs of residents at The Moorings. Ensuring quality of resident services, meeting or exceeding community quality and service standards.
REPORTS TO: Director of Social Services
SUPERVISES: May lead/supervise paraprofessionals.
DUTIES AND RESPONSIBILITIES:
- Provide counseling and support to residents, families, and friends of residents in individual settings
- Function as an integral member of the multidisciplinary team; collaborate and provide clinical consultation as needed.
- Assist residents in obtaining services that enhance their ability to function at the highest possible level of independence.
- Maintain and protect the confidentiality of resident information.
- Foster effective communication and teamwork among residents, families, and staff.
- Develop and present educational and psychologically supportive programs and for residents
- Provide appropriate psychosocial assessments and documentation in resident records in compliance with federal and state guidelines.
- Maintain communication with families of residents to inform them of any special requirements that a resident may need.
- Inform individuals of their rights and responsibilities as residents of Presbyterian Homes and serve as resident advocate.
- Participate in interdisciplinary committees focusing on the resident’s health care needs.
- Develop and maintain community resource information lists.
- Promote the Presbyterian Homes’ vision, mission and values.
- Participate in professional seminars and workshops to maintain LCSW licensure.
- Is aware of Presbyterian Homes Corporate Compliance policies, including those having to do with confidentiality of information and concern for the well-being of residents.
- Demonstrates behaviors and completes work in a manner consistent with these policies.
- Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and/or clinical documentation.
- Responsible to use computers and software as required by work environment in order to enhance communications, performance and knowledge.
EDUCATION:
Bachelor’s degree required. Master’s degree in Social Work preferred.
EXPERIENCE:
Two years relevant post-graduate work. Geriatric experience desired.
QUALIFICATIONS:
- LSW/LCSW license by the State of Illinois Preferred
- Demonstrated ability to communicate in a clear, articulate manner; both verbally and in writing.
- Well-developed counseling skills.
- Awareness of resident needs as a whole person (social, physical, spiritual, and intellectual).
- Willingness and desire to work with older adults and a clear understanding of the aging process.
- Proactive personality with an emphasis on teamwork and flexibility. Must be able to work in an unstructured environment, handle several tasks simultaneously, solve problems and respond quickly and effectively.
- Ability to present information to group settings.
- Computer literacy and public speaking skills.
- Capable of independent judgment, initiative and ingenuity.
CONTACTS:
Staff of all departments, residents, family members of residents, friends of residents, and community support services. Maintain community relationships with key local leaders.
